That was the original record for for your Jeep and it recorded all work done on it. It would have been destroyed upon sale of your Jeep by the Property Disposal Officer. Someone was offering copies of the forms but it has been a few years.

Since correcting Bendy Me Over is almost a full time job, I'll just post the page from oh, just about any MV TM9:

Its for major assemblies. Read the paragraph and you'll learn. Since Bent Out of Shape Dover doesn't own any TMs he most likely missed the directions of use... oh wait, they are also PRINTED ON THE FROM ITSELF. hahahaha

Note the last line:

Even if you graduate last in your class... you still graduate. hahahahaha
